Can 12 AWG ever go on a 25 A or 30 A breaker?
Not for ordinary branch circuits. NEC 240.4(D)(5) limits 12 AWG copper to 20 A overcurrent protection regardless of the wire's insulation rating. The narrow exceptions (motor circuits, certain HVAC nameplate rules under 240.4(G)) are equipment-specific and don't apply to receptacle or lighting circuits.
Is 3.3 % voltage drop a code violation?
No. The 3 % branch-circuit figure appears in an Informational Note to NEC 210.19(A), which is advisory. The practical effects are real, though: at full load this run delivers about 116 V, motors run slightly hotter, and resistive loads lose about 3 % of their output. For a lightly loaded receptacle circuit it is a non-issue; for a sustained near-capacity load, one gauge up (10 AWG) cuts the drop to about 2.1 %.
Does the 50 ft in this check mean distance from the panel?
It is the one-way length of the cable path, not the straight-line distance. The formula already accounts for current flowing out and back (the factor of 2 in 2 × K × I × L ÷ CM). Cable routed up a wall, across a ceiling, and back down commonly adds 20–40 % over the floor-plan distance, so measure the actual run.
